Four examples of the custom-bodied Ferrari 330 GTC Speciale were built by Carrozzeria Pininfarina. These hand-built Speciales were given custom coachwork that rested on the Ferrari 330 GTC gran turismo platform.

Introduced at the 1965 Geneva Auto Show, the GTC was a two-place coupe designed and built by Pininfarina. They had refined and modern styling, with impressive performance and luxurious appointments. The Speciale went further, with a clean-sheet body style that incorporated many Pininfarina cues with many later finding their way into future production models. In the front was a nose that was similar to the limited-production 365 California Spider, with covered headlights, split bumpers, and a classic egg crate grille. The first two examples had retractable driving lights. The Speciale's engine bay louvers were placed on either side of the bonnet's trailing edge - a design feature that would later be implemented on the 365 GTC. The long silhouette and ample glass greenhouse were undoubtedly inspired by the mid-engine show cars of the era. The rear quarter window shape were later used on the 365 GT 2+2. The curved rear glass of the Speciale had first been seen on the 206 Dino displayed at the 1965 Paris Auto Show, and later repeated on the two 'Tre Posti' 365 P Berlinetta Speciales. The abrupt Kamm tail design was similar to the 365 California and mid-engine show cars.

The Ferrari 330 GTC Speciale was unveiled at the Brussels Motor Show in 1967. The first example, chassis 09439, was sold to a royal customer – Princess Liliane de Réthy of Belgium. The second example, chassis 09653, was built for Dr. Michael DeBakey of Houston, Texas. The third example was chassis 10107, Pininfarina job number 99686, and given body number C0260. Its first owner was Maria Maddalena Da Lisca.

This particular example is chassis number 9653, the second example built. Dr. DeBakey accompanied King Leopold and Princess de Réthy on a visit to Maranello. While touring the Ferrari factory, he placed an order for the 330 GTC Speciale. Work began in August of 1966. It was internally designated by job no. 99676 and received the hand built body number C0174. It was finished in Azzurro Aurora (Aurora Blue) with black leather upholstery, blue cloth seat inserts, and blue carpeting. It was originally equipped with air-conditioning, power windows, instrumentation in miles, a chromed front grille guard, and Borrani wire wheels.

Prior to being delivered to Dr. DeBakey, the Speciale was retained for promotional duties. It appeared in several press photos, and was put on Pininfarina's stand at the 37th annual Geneva Auto Show held in March 1967. Other cars displayed at the Pininfarina stand included a 365 California Spider and one of the three-seat 365 P Berlinetta Speciales.

Dr. DeBakey retained the car for just a year before gifting it to his brother Dr. Ernest DeBakey of Mobile, Alabama. Carl De Bickero of Chicago purchased the Speciale in 1973, overhauled the car's engine, and then sold it, in January 1974, to Norman Silver of High Point, North Carolina. It remained with Mr. Silver until 1983, when it was sold to John Mecom Jr. of Houston. In 1991 it was sold to Peter Sidlow of Los Angeles. Shortly after acquiring the car, Mr. Sidlow commissioned a complete, concours-quality restoration. After the work was completed, it earned several important honors, including First in Class and the Luigi Chinetti Award at the 1992 FCA Annual Meet and Concours as well as a First in Class at the FCA Vintage Ferrari Concours at Concorso Italiano in August 1992.

The current owner acquired the car in 1993. It has seen little use over the past two decades and has not been exhibited in any major concours d'elegance since 1992.

The car is powered by a 3,967cc SOHC V-12 engine fitted with three Weber 40 DFI carburetors and delivers 300 horsepower. There is a five-speed manual transaxle and four-wheel Girling disc brakes.
